The Groupe Dentaire La Vallée must collect the personal information necessary for providing dental care through the attached confidential medical-dental questionnaire, which is presented to you for signature. The confidential medical-dental questionnaire is kept in your dental file, and your dental file is stored on digital media with access controlled by access codes. Only healthcare personnel will have access to your health information, and administrative personnel will only have access to the information necessary for their duties (billing, appointment scheduling, communication with insurance companies).
